Dr. Marcello Guglielmi is a Board Certified Oral and Maxillofacial surgeon. Only Oral and Maxillofacial (OMS) surgeons have a wide surgical knowledge of the temporomandibular joint (TMJ). Early detection, diagnosis and treatment by an experienced specialist are very important, as left untreated, TMJ and facial pain can lead to more serious conditions.

TMJ disorders develop for many reasons such as trauma, habitual jaw clenching and teeth grinding and arthritic conditions that affect your joints. The origination of TMJ disorders can also be unexplained. Treating TMJ

TMJ Splint: Night Guard

Dr. Guglielmi may determine treating you with a TMJ splint, commonly referred to as a custom-made night guard can be beneficial to those suffering from pain associated with a temporomandibular joint disorder to align your bite with the goal of reducing pain and increasing jaw function due to misalignment.
A TMJ splint night guard is custom made for you by an outside laboratory that specializes in these very specific appliances. The material is thicker than an orthodontic retainer and will fit tighter, as it should, than any over the counter night guards. These appliances are specially made to protect your teeth from excessive grinding and clenching.

What to expect:

  • Full Comprehensive Exam 
  • X-rays taken
  • A model will be taken of your mouth and sent to the lab
  • You will return to our office when your night guard arrives for a fitting
  • Adjustments to your night guard will be performed by Dr. Mansour or Dr. Sanchez to provide a precise fit
  • You will wear your night guard every night
  • Some patients wear the night guard for a few months, while others will wear it long term

Orthodontic Retainer

In some cases, Dr. Guglielmi, Maxillofacial Surgeon may determine an orthodontic retainer, called an Essix retainer, will be the best option to protect a patient’s teeth when mild TMJ symptoms exist. An Essix retainer is not an option if you are a severe teeth grinder. Dr. Mansour, our expert in orthodontics will create a mold of your teeth, make your retainer in his in-house Sunshine Smiles Lab and adjust your retainer for a perfect fit.

TMJ Surgery

TMJ & Facial Pain Nightguards Splinting Diagnostic

Surgical intervention is considered a last resort for chronic TMJ disorders. Temporomandibular Joint disorders are commonly muscular related issues that do not require surgical intervention but do require an expert to oversee, patience and being open to trying different, non-invasive approaches to reduce inflammation and overall tension.
In certain cases, Dr. Guglielmi will recommend less invasive surgical intervention.

Arthrocentesis

A minimally invasive form of treatment, arthrocentesis involves flushing the joint space with a sterile solution, and in some cases, injecting a corticosteroid or lubricant to enable easier, pain-free movement. Arthrocentesis is performed by an expert in Oral and Maxillofacial surgery

TMJ Self-Care

Taking care of yourself on a daily basis is especially important when you suffer from TMJ pain. Constant TMJ pain can cause patients to become overly stressed, sleep deprived and, in some cases, emotionally distraught. It is imperative that you seek the professional help you need and commit to daily self-care to manage the effects of mild, moderate or severe TMJ pain. No matter what level you are at, attention to your health is required.

Common self-care techniques may include:

  • Hot or cold compresses
  • Massage
  • Over the counter anti-inflammatories
  • Stretching the jaw
  • Eating softer, easy to chew foods
  • Avoiding harder, crunchy foods
  • Stress management
  • Relaxation techniques
